Paediatrics


Introduction
  • Govt Medical College Hospital, Thiruvallur is situated along the Chennai – Tirupati National Highway and close to Chennai – Bangalore National Highway. This is the only tertiary care pediatric facility with NABH and MusQan accreditation catering to the population of about 37 lakhs from 3 districts and border areas of Andhra Pradesh. Most of the Neonates and Pediatric patients from in and around Thiruvallur district referred to this Hospital which has an established Special Newborn Care Unit and Pediatric Resuscitation and Emergency Medicine Department , where all the primary stabilization and emergency services are done as per the NNF and PREM protocol.
  • At present Dept of Pediatrics at Govt Medical College Hospital, Thiruvallur is one of the most established departments in the Hospital with all the faculty and infrastructure facilities doing all types of emergency management of common pediatric emergencies like poisoning and envenomation, tropical diseases like Dengue, Scrub Typhus, Malaria, Sepsis with shock, Viral hepatitis. Our ICUs are delivering invasive and non invasive ventilator care like HHHFNC, CPAP support and performing procedures like securing central venous line, intercostal drainage, umbilical vein catheterization, endotracheal intubation, surfactant administration and double volume exchange transfusion for neonatal jaundice .
  • Government Medical College and Hospital, Thiruvallur is the only tertiary care centre for the entire Thiruvallur district and adjacent 2 districts of Kancheepuram, Ranipet, Chengalpet and adjoining border areas of Andhra Pradesh, offering tertiary level Pediatric care.

Infrastructure
  • Department of Pediatrics has an exclusive Outpatient department running Well baby clinic, Immunization clinics, specialty clinics , IYCF counseling room with nearby play area for children attending our outpatient clinic. The department has well equipped Neonatal and Pediatric Intensive Care Units with laminar airflow. In Pediatric OPD and TAEI complex we have separate Pediatric Resuscitation and Emergency Medicine (PREM) units and the state of the art -Pediatric Intensive care Unit (PICU) in 1st floor of Main Block and Sick Newborn Care Unit (SNCU) in the 1st floor of Maternal and Child Health (MCH) Block. The Department has an exclusive Pediatric ward with 2 units in the 4th floor of the Main block. We have a fully functional District Early Intervention centre (DEIC) in our hospital campus.

Pediatric Out Patient Clinic

  • Our outpatient clinic caters to needy and poor children with common ailments not only from Thiruvallur district, but also from nearby districts like Kanchipuram, Ranipet and Chengalpet as well as neighboring state- Andhra Pradesh.
  • The department conducts various specialty clinics like well baby clinic and High Risk Newborn clinic, Immunization clinic, Nutrition clinic including IYCF services, Pediatric Cardiology, Pediatric Neurology, Pediatric nephrology clinic, Pediatric Endocrinology clinic, Asthma Clinic, Child Guidance Clinic and Hemophilia day care centre. An Assistant Professor has been posted as in-charge of each clinic and is monitored regularly by Head of the Department of Pediatrics.

Pediatric Resuscitation and Emergency Medicine (PREM)

  • Our Pediatric department caters Emergency pediatric medical and trauma care services PREM Units established in Pediatric OPD and TAEI Building. Round the clock pediatric emergency care services are provided by the duty Pediatrician and Pediatric Residents in the PREM units wherein sick children brought with various complaints are triaged and managed appropriately as per PREM protocol.

Pediatric Intensive Care Unit (PICU)

  • We have a 13 bedded Pediatric Intensive Care Unit (PICU) where the sick children are admitted after initial triage and stabilization in the PREM unit. Children brought with respiratory and cardiac emergencies, status epilepticus, shock, poisoning and envenomation, submersion injury and burns are treated by our team of trained intensivists and staff nurses. Our PICU is equipped with state of art facilities for Paediatric Invasive and non invasive ventilation and HHFNC facilities for infants and young children .
  • We are equipped with Bedside X ray unit and Advanced Cardiac and Ultrasonogram machines which play a vital role in early identification of various respiratory, cardiac and abdominal emergencies in children admitted in our PICU.

Special Newborn Care Unit (SNCU)

  • The Special Newborn Care Unit (SNCU) at Government Thiruvallur Medical College Hospital provides specialized care for critically ill and premature newborns. Equipped with overhead radiant warmers and incubators, ventilators and monitoring systems, the NICU ensures advanced care for neonates with low birth weight, respiratory distress, sepsis, and other complications. A dedicated team of pediatricians and staff nurses offers round-the-clock care, focusing on stabilizing and nurturing newborns. Our services include level 3 SNCU care including ventilator and CPAP support, phototherapy, exchange transfusion, kangaroo mother care (KMC), and nutritional support, ensuring holistic management. Our SNCU is equipped with Bedside X -ray unit and Advanced Cardiac and Ultrasonogram machines through which we are able to identify various respiratory , cardiac and surgical emergencies in babies treated in our SNCU. The SNCU at Thiruvallur Medical College plays a pivotal role in reducing neonatal mortality and improving health outcomes for vulnerable newborns in the region.

Human Milk Bank

  • Our department also runs a round the clock functioning Human Milk Bank where the milk expressed from the donor mothers is stored and also dispensed to the premature and sick babies admitted in our hospital. At any given point of time our milk bank can store around 15 liters of donated milk from postnatal mothers.

Pediatric Ward

  • The Pediatric Ward with 80 beds at Government Thiruvallur Medical College Hospital is dedicated to provide comprehensive care for children from infancy to adolescence. It serves as a crucial hub for addressing a wide range of pediatric health conditions, including infectious diseases, nutritional deficiencies, disorders of growth and development and various systemic illnesses. The ward is staffed with a team of skilled pediatricians, staff nurses and support staff committed to ensure the well-being of young patients.
  • Services encompass diagnosis, treatment, and monitoring of conditions like respiratory infections, gastrointestinal disorders, fevers, and congenital anomalies. Special attention is given to nutritional rehabilitation for malnourished children and counseling for families on maintaining child health. The ward actively participates in government health programs such as immunization drives, anemia control, and deworming campaigns. A child-friendly environment (play area and child friendly toilets)is maintained to make hospital stays less intimidating for young patients.
  • The Pediatric Ward at Thiruvallur Medical College Hospital plays a vital role in ensuring accessible, affordable, and quality healthcare for children in the district, significantly contributing to improve pediatric health outcomes.

District Early Intervention Centre (DEIC)

  • The District Early Intervention Centre (DEIC) is a vital component of the Rashtriya Bal Swasthya Karyakram (RBSK) in Thiruvallur, aimed at ensuring early identification and intervention for children with developmental delays and disabilities. Located as a separate unit, DEIC provides free diagnostic and therapeutic services for the "4 Ds" – Defects at birth, Diseases, Deficiencies, and Developmental delays. Services include physiotherapy, occupational therapy, speech therapy, and nutritional support, delivered by a multidisciplinary team of Pediatrician, therapists, and counselors. There is also a separate pediatric dental unit in our DEIC.
  • DEIC regularly conducts ROP screening for premature babies, OAE screening for all newborns born in the institution and cardiac camps every fortnight. Growth hormone injections are regularly supplied to children with short stature on needed basis under The Comprehensive CM Health Insurance Scheme.

Our Department of Pediatrics facilities teaching of

Undergraduate MBBS teaching programs

  • Our Department of Pediatrics conducts theory and practical demonstration classes for Second year and Pre final year MBBS Students as per NMC Medical education unit guidelines.
  • Our Government Thiruvallur Medical College, Thiruvallur has 100 Undergraduate seats each academic year starting from 2022. At present we are having First year, Second year, Final year part-1 & 2 students in our college.
  • In the morning, In OPD and Wards we are facilitating undergraduate students as per NMC guidelines. As per COMPETENCY BASED UNDERGRADUATE CURRICULUM we provide education and clinical skill knowledge through various means from first year to final year
  • Early Clinical Exposure (ECE) is a learning tool that provides medical students with practical experience in a healthcare setting early in their studies
  • Competency-based curriculum: The curriculum is designed to help students develop competencies in working in a healthcare team, practicing medicine, and more
  • 2 weeks Clinical classes are held in second and prefinal year where proper schedule is based on Curriculum including case discussion, self directed learning and small group teaching.
  • A summative assessment is made at the end of session testing their competency skills which helps in strengthening their weak areas.
  • Theory classes were held regularly every month for prefinal years based on Cbme curriculum that includes pediatrics & procedural skills, AETCOM modules , Integration topics.

C.R.M.I. from Foreign Medical Graduates

In our Government Medical College and Hospital, CRMI from Foreign Medical Graduates are posted after completing their Foreign Medical Graduate Examination for past 2 years with each academic year approximately 100 CRMI graduates.


We are teaching basic clinical exposure and hands on training to all the posted CRMIs to enable them to diagnose and treat common pediatric ailments.


All the posted CRMIs are allowed to assist in

  • Securing intravenous line
  • Intravenous blood sampling
  • Lumbar puncture for CSF analysis
  • Newborn Resuscitation
  • Basic Newborn care
  • Kangaroo Mother care
  • Pediatric resuscitation including airway management, assisted ventilation and shock management

CRMIs are allotted topics for case presentation and symposium during their postings. For all CRMIs, evaluation of their knowledge and clinical skills is done at the bedside periodically and at the end of their posting, assessment is conducted.


District Residency Program (DRP) for Post graduates:

As per Post Graduate Medical Education Regulations 2000 (PGMER-2000) vide notification dated 16.09.2020, Post graduate students in various Government and Private Medical Colleges in their second year are posted in New Medical colleges, District Headquarters Government Hospitals and Taluk Hospitals.


In our Government Medical College and Hospital, Thiruvallur, District Residency Program Post Graduates are regularly posted for the past two years from various Government and Private Medical Colleges. All the DRP Post graduates posted in our Department are trained under subject specific learning objectives. This will be dealt with under the following headings:


  • Theoretical knowledge (Cognitive domain)
  • Practical and clinical skills (psychomotor domain)
  • Attitudes including communication skills (Affective domain)
  • Writing thesis / Reviewing Research activities (Scholarly activity)
  • Training in Research Methodology (Practice based learning, Evidence based practice)

All the DRP PGs are involved in the following academic activities

  • Case Presentation- Once a week in the ward and in Outpatient department
  • Seminars- Once a week on every Friday
  • Journal club- Once a week on every Saturday
  • Academic grand ward rounds- Twice a week presentation of cases by residents and clinically applicable discussions
  • Quality of training are monitored by log books, supportive supervision and continuous assessment of performance.

Publication of Textbook Oct 21,2022 Dr. Anupama S., who was working as Senior Resident in the Department of Pediatrics, has authored a comprehensive book for MBBS and postgraduate pediatric students, published by Elsevier, an international publisher. The book was officially released by the Honorable Minister for Health and Family Welfare of Tamil Nadu by Thiru. Ma. Subramanian on Oct 21,2022.
