General Medicine


Introduction
  • Department of General Medicine has an exclusive Outpatient clinic situated in the first floor of the Main block with separate Male and Female OPDs. Male and Female General Medicine wards, IMCU and ICCU Ward situated in the 3rd floor of the Main block. IMCU (ICCU) has 20 beds with all life saving necessary equipments.
  • Round the clock emergency care services are provided by the duty Assistant Physician in the Emergency Department.
  • We, the Department of General Medicine treat patients on OP and IP (inclusive of emergencies) basis. Per day we treat around 800 to 900 out patients. Per day we admit around 60 to 80 Inpatients. We treat emergency cases such as MI, CVA, poisoning, Encephalopathy etc.. according to fixed protocols. We have injection OP in OPD premises and we administered injections for Dog Bites, TT Vaccination and for Minor ailments on regular basis.
  • We conduct specialty clinics like Hepatitis clinic every Thursday where we give treatment as well as follow up care for hepatitis B and C patients. In the 1st Floor of the Main block we also have NCD Clinic were in we treat patients with diabetes, dyslipidemia, systemic hypertension etc..and dispense drugs on monthly basis. We have separate Male and Female ECG rooms near to OPD.
  • Male Medical Ward consists of 104 Beds with adequate space, Female Medical Ward consists of 72 beds, IMCU(ICCU) consists of 20 beds, IMCU equipped with 12 ventilators and other lifesaving equipment’s round the clock food and water services are available for the inpatients, rest rooms available for both patients as well as their care takers. Wheel chairs and stretchers available for mobilizing the sick patients.

  • Procedure Room - male medical ward ,female medical ward and IMCU has got separate procedure room with bed facility, for performing various procedures such as pleural fluid taping, ascitic fluid taping, bone marrow biopsy etc. Aseptic precautions adhered to. Monitoring facilities available, emergency drugs available.

  • Xray Facility – Mobile X ray machine available in IMCU, X ray lobby available in male medical ward Female Medical ward.

  • USG and 2D Echo Facility – Mobile scan machines available in IMCU for the benefit of sick patients

  • Demonstration room for undergraduates and Postgraduates – Area of 1100 sqft area with seating capacity for 35 students with White board and xray lobby.

  • There is an exclusive Demonstration room near our ward for postgraduate and undergraduate students along with Audiovisual aids, like Overhead LED projector, screens, Amplifier, Mike, speaker and Digital Xray lobbies for demonstration purposes.

  • Wards - Our Department of General Medicine has exclusive Male and Female Medicine ward with total bed strength of 176 beds monitored round the clock by the Duty Assistant Physician and Duty staff nurses with Male Medicine ward strength of 104 beds and Female Medicine ward strength of 72 beds and 20 ICU beds. Each ward have separate nursing station, dressing room, Demonstration room, pantry, lab sample collection centre, chief and faculty rooms, record and store rooms and Office rooms with two exclusive ward managers for CMCHIS scheme. Most of the Major and minor cases are operated under Comprehensive Chief Minister Health insurance scheme (CMCHIS) whereby preauthorisation approval and claims are regularly monitored along with two ward managers.

  • There is total of 5 rooms for faculty with each area of 200 sqft for Head of the Department, Associate professor, Assistant Professor And Resident doctors.

  • Skills lab – For the benefit of post graduates and to impart hands on training in various surgical procedures, an exclusive skills and research lab is being established with all the necessary equipments/ instruments. Apart from this there is an exclusive skills lab for Thiruvallur Medical college for purpose of BLS and ALS training for under and postgraduates

  • Department library – There is an exclusive department library with adequate number of necessary standard textbooks in General Medicine and national and international journals as per the NMC norms.

  • Apart from this there is a central library in the college campus with necessary books and journals. The department has an access to various e-journals in General Medicine from the Tamil Nadu Dr. M.G.R. Medical University Central Library by e-consortium.

DIALYSIS WARD

Dialysis ward consists of 25 beds,15 dialyzer machines.4 sessions (slots) available in a day around 40 to 45 Chronic Kidney Disease patients get their dialysis sessions done per day round the clock the sessions are monitored by Doctors, staff nurses and technicians. Per month more than 1000 Hemo Dialysis cycles are done at dialysis ward. Injections Iron Sucrose weekly once given to CKD Patients with HB less than 8 grams. Injection Erythropoietin given to CKD patients every fortnight. Nonstop RO Supply available in Dialysis area.

    CHEST MEDICINE

  • At the department of TB and Respiratory Medicine services in regard to TB and general pulmonology diseases are available both on OP as well as IP bases, with facilities to administered intensive respiratory care to needed patients. The OP department is supported with diagnostic facilities – Sputum examination, Culture and sensitivity studies, PFT and Imaging studies special clinics are conducted for diagnosis treatment and patient education for chronic lung diseases like COPD and asthma.

  • Tobacco cessation clinic is also conducted in coordination with department of psychiatry and Community Medicine. The district TB Cell is attached to our department. TB services – screening, diagnosis, treatment(DOTS) and follow-up of patients with TB and EPTB are undertaken on OP and IP basis there is an exclusive well ventilated Tb ward for Male and Female with facilities to manage seriously ill B patients(TNKET patients).

  • The General Medicine PGs will have adequate training in management of respiratory illnesses. The average monthly OPD census is around 2250 and IP census is around 65 further the Post Graduate will have opportunities to undertake research activities in respiratory Medicine with available resources.


DERMATOLOGY

At the Department of Dermatology per day around 200 patients are treated on OP basis and around 05 to 10 patients are admitted as IP. In dermatology ward around 10 beds available procedures such as PRP, SSS, Fungal KOH mount/Scrapings, Intralesional Steroids, Salicylic acid treatment, Wart PRP, Electro Cautery, MC needling, biopsies etc. are carried out on daily basis.


ART CLINIC

At the department of ART around 50 patients are treated on OPD basis regularly patients tested for various sexually transmitted diseases such as HIV, VDRL etc. Patient diagnosed with AIDS are provided with anti-retro viral drugs on monthly basis (patients with comorbidities) and 3 months once (patients without comorbidities)


PSYCHIATRY

At the department of psychiatry around 50 patients are treated on OPD basis there are 30 beds available in psychiatry ward patients diagnosed with psychotic disorders are given ECT.


CARDIOLOGY

Department of Cardiology is situated in 2nd Floor Main block OPD clinic conducted on Tuesdays, thursdays and Saturdays on OPD days cardiologist treats OPD patients and does 2D echo, TMT exclusively for OP patients on Mondays, Wednesdays and Fridays cardiologists treats IP patients and does 2D echo for the same. Around 150 OP patients are treated.


NEUROLOGY

Department of neurology situated in 1st floor main block. Neurologist runs OP Clinic on Mondays, Wednesdays and Fridays. Neurologist visits IP patients in various wards and gives opinions on Tuesdays, Thursdays and Saturdays. EEG and NCS are done by neurologist for needful patients around 100 to 120 patients are seen and treated on OP basis.

    Undergraduate MBBS teaching programs

  • Our Department of General Medicine conducts theory and practical demonstration classes for Second year and Pre final year MBBS Students and Final year as per NMC Medical education unit guidelines.

  • Our Government Thiruvallur Medical College, Thiruvallur has 100 Undergraduate seats each academic year starting from 2022. At present we are having First year, Second year , pre final year and final year students in our college.

  • In the morning, In OPD and Wards we are facilitating Early Clinical exposure as per NMC guidelines for Second year pre final year and final year students.

  • As per COMPETENCY BASED UNDERGRADUATE CURRICULUM we provide education and clinical skill knowledge through various means from first year to final year.

  • Early Clinical Exposure (ECE) is a learning tool that provides medical students with practical experience in a healthcare setting early in their studies.

  • Competency-based curriculum: The curriculum is designed to help students develop competencies in working in a healthcare team, practicing medicine, and more.

  • Clinical classes are held in second year, prefinal year and final year where proper schedule is based on Curriculum including case discussion, self directed learning and small group teaching

  • A summative assessment is made at the end of session testing their competency skills which helps in strengthening their weak areas

  • Theory classes were held regularly every month for prefinal years based on Cbme curriculum that includes General Medicine & procedural skills, AETCOM modules , Integration topics


    C.R.M.I. from Foreign Medical Graduates

  • In our Government Medical College and Hospital, CRMI from Foreign Medical Graduates are posted after completing their Foreign Medical Graduate Examination for past 2 years with each academic year approximately 100 CRMI graduates.

  • We are teaching basic clinical exposure and hands on training to all the posted CRMIs to enable them to diagnose and treat common General Medicine ailments. Each CRMIs are trained to have ability to diagnose treat various medical diseases.

  • All the posted CRMIs are allowed to assist in:


  • Endotracheal tube insertion

  • Pleural Fluid tapping

  • Ascitic Fluid tapping

  • Bone marrow aspiration and biopsy

  • Lumber Puncture etc


    District Residency Program for Post graduates

  • As per Post Graduate Medical Education Regulations 2000 (PGMER-2000) vide notification dated 16.09.2020, Post graduate students in various Government and Private Medical Colleges in their second year are posted in New Medical colleges, District Headquarters Government Hospitals and Taluk Hospitals.

  • In our Government Medical College and Hospital, Thiruvallur, District Residency Program Post Graduates are regularly posted for the past two years from various Government and Private Medical Colleges.

  • Theoretical knowledge (Cognitive domain)

  • All the DRP Post graduates posted in our Department are trained under subject specific learning objectives. This will be dealt with under the following headings:

  • Practical and clinical skills (psychomotor domain)

  • Attitudes including communication skills (Affective domain)

  • Writing thesis / Reviewing Research activities (Scholarly activity)

  • Training in Research Methodology (Practice based learning, Evidence based practice)

  • Professionalism

  • Teaching skills

  • All the DRP PGs are involved in the following academic activities


  • Case Presentation- Once a week in the ward and in Outpatient department

  • Seminars- Once a week on every Friday

  • Journal club- Once a week on every Saturday

  • Academic grand ward rounds- Twice a week presentation of cases by residents and clinically applicable discussions

  • Quality of training are monitored by log books, supportive supervision and continuous assessment of performance.
